Bifidobacterium lactis BB-12
The most-studied Bifidobacterium — gentle gut regularity and infant comfort
What is Bifidobacterium lactis BB-12?
Bifidobacterium lactis BB-12 (Bifidobacterium animalis subsp. lactis BB-12) is a probiotic strain used for increases defecation frequency in adults with low frequency and mild abdominal discomfort (regularity). NutriDex grades the human evidence as Moderate. Bifidobacterium animalis subsp. lactis BB-12 is the world's most-studied Bifidobacterium, with randomized, placebo-controlled trials supporting modest, strain-specific benefits for bowel regularity and infant comfort. The largest probiotic regularity trial ever published (Eskesen 2015, n=1248) showed BB-12 significantly raised defecation frequency in adults with low frequency and abdominal discomfort, and a colic RCT (Nocerino 2020) found 80% of breastfed colicky infants on BB-12 achieved a >=50% reduction in crying versus 32.5% on placebo. Mechanistic and infant-infection data are supportive but more preliminary, and evidence for preventing antibiotic-associated diarrhea is still emerging rather than definitive for this specific strain.

The same supplement can be well-proven for one use and unproven for another — here is the human evidence graded outcome by outcome.
| Outcome | Evidence | Effect | Studies |
|---|---|---|---|
| Defecation frequency / regularity (adults)Large RCT (Eskesen, n=1248) raised defecation frequency; single trial but well-powered and strain-specific. | Moderate | ↑ benefit · small | 1 |
| Infant colic crying timeOne RCT (Nocerino, n=80 breastfed): 80% vs 32.5% achieved >=50% crying reduction; needs replication. | Preliminary | ↑ benefit · large | 1 |
| Respiratory tract infections (infants/adults)Two RCTs (Taipale infants, Meng adults) reduced RTI occurrence; small and population-specific. | Preliminary | ↑ benefit · small | 2 |
| Microbiome preservation during antibioticsOne RCT (Merenstein) preserved fecal SCFA; surrogate endpoint, not clinical AAD. | Preliminary | ↑ benefit · small | 1 |
| Blood lipidsCrossover RCT (Matsumoto, n=30) found no change in cholesterol or triglycerides. | Preliminary | — no effect · negligible | 1 |
