Iron vs B12: Which Deficiency Is Making You Tired?

Iron and vitamin B12 are the two big deficiency causes of fatigue and anemia, and people constantly supplement one when they need the other — or need neither. Both grade strong for correcting their own deficiency, but they cause different anemias (iron: small pale red cells; B12: large immature ones), have different risk groups, and different stakes: untreated B12 deficiency can cause irreversible nerve damage, while blind iron supplementation carries real overload risk. Suspect iron if you menstruate heavily, are pregnant, donate blood, or eat little meat: ferritin is the key test, and correcting deficiency (typically 65–100 mg elemental iron every day or alternate days, with vitamin C) reliably resolves the fatigue and anemia it caused. Suspect B12 if you are vegan or vegetarian, over 60, on metformin or acid-suppressing drugs, or have gut disease: deficiency causes fatigue and macrocytic anemia but also neuropathy — numbness, tingling, balance and memory problems — which can become permanent if missed; oral 500–1,000 mcg/day works for most, injections for absorption disorders. Two safety rules: never take long-term iron without a confirmed deficiency (excess iron is genuinely harmful and can mask GI bleeding as a cause), and never let folic acid 'fix' an anemia without checking B12, since it can hide the blood picture while nerve damage progresses. Should I take iron or B12 for tiredness?
